Information for New Clients

If you are a new Home Pet Vet client, please fill out a:

1. New Client Profile Form
   
   » Online and submit

   AND

2. For each pet, complete a Pet Health History Form
   
   » Online and submit

OR

1. New Client Registration Form/Pet History Form (combined)
   
   » PDF version to print, sign and fax to 541-899-5987
2. For each additional pet, please fill out an additional form (Pet History part only)

Note : If you are unable to submit either form, we will carry extra copies with us to be completed and signed at your home.

3. If you completed the forms online, we will contact you to confirm your pet information and answer any questions.
4. If convenient, please gather any previous medical information & records for review during our home visit. While this is very helpful, it is not required.
5. Please place a note on the door if you prefer that we do not ring your doorbell.
